I, the undersigned, do hereby testify that I am not under the influence of alcohol or drugs. I am 18 years of age or older. I do not have AIDS, hemophilia, hepatitis, or any other blood transmitted diseases. I agree to the following aftercare procedures as listed to me by Lady Magnolia Tattoo. Any infection or medical problems arising from my decision to get a tattoo at this time are fully my responsibility both financially and legally, and to my own negligence and not the following aftercare procedures. Lady Magnolia Tattoo and its employees and representatives cannot be held liable for any legal action resulting from my decision to get a tattoo at this time. Any touch up work due to my own negligence will be at my own expense. With the application of a tattoo, there is a possibility of discomfort or pain, and risk of infection. The markings of a tattoo are permanent. In some cases, the possibility of an allergic reaction to the pigment or other materials used, may occur. If so, contact Lady Magnolia Tattoo. The client shall consult with a healthcare practitioner at the first sign of infection or allergic reaction, and report any diagnosed infection, allergic reaction,or adverse reaction resulting from the tattoo to the artist and to the Texas department of state health services, drug and medical devices group, at 1-888-839-6676


Keep bandage on overnight. 2. After removing bandage, wash tattoo with anti-bacterial soap and hot water. 3. Wash your tattoo 3-5 times a day for the first week. 4. After second day of washing you can start applying a thin layer of antibiotic ointment or cream and use sterile bandages or other sterile dressings when necessary. Make sure to keep your tattoo moisturized daily for the first month. Avoid the sun and pools for the first 2 weeks.
